In the previous version where you could use mp3 files, you could reliably remix the mp3 file to have a lower volume.
On the latest version where MP3 support is removed, it seems to be the case that the file you provide is played back at a higher volume. The lower the audio level in the wave file, the worse the quality of the file is.
The volume does not seem to change regardless of the source's volume; though, as mentioned, changing the source volume affects the quality of the end output (turning it down too low results in pure static at 100% volume).
Not sure if 3CX provide an FAQ on how to reduce volume for MoH files?

Volume issues are subject to the way that audio files are recorded, modified, and saved.
If files are saved in WAV, PCM, 8 KHz, 16 bit there should be no problems.
If there are still problems, then the installed codecs on the computer must be investigated.
